POSITION SUMMARY


The Licensed Clinical Social Worker provides direct and indirect care to a specific patient population using social work assessment, high risk screening and counseling skills along with age appropriate interventions to facilitate appropriate use of healthcare resources and adjustment to illness. The Licensed Clinical Social Worker manages patients across the health care continuum to achieve optimal clinical, financial, operational and satisfaction outcomes and facilitates timely discharge planning. The Licensed Clinical Social Worker promotes interdisciplinary collaboration in all aspects of work, participates in individual and group care conferences facilitating patient satisfaction, safe discharges and timely dispositions. The Licensed Clinical Social Worker provides patients/families post acute options for alternative levels of care, completes appropriate referrals for needed resources as well as develops and executes the discharge plan.


REQUIREMENTS


Must have 1-3 yrs of experience as a social worker in a medical setting. MSW degree from a school of social work accredited by the Council of Social Work Education. Current California license issued by the Board of Behavioral Sciences. Los Angeles City Fire/Safety Card within 30 days of hire. Current BLS Card issued by American Heart Association.
