Position Description:


Assoc Clinical Admin Coordinator is responsible for the completion of set processes and protocols. Assists Primary Care Teams with monitoring patients in care management or the referral process. The Patient Care Coordinator interacts with other team members concerning patient needs identified during the course of day-to-day activities. May adapt procedures, processes and techniques to meet the moderately complex requirements of the position.Positions in this function are responsible for checking in, scheduling appointments, answering billing questions and managing patient medical records for a clinic. Roles may vary based on the size of the clinic(s).

Primary Responsibilities:


Follow up to ensure patients arrive for post hospital visits.

Monitors chronic disease and high-risk patients to ensure Primary Care Physician visits are kept.

Reviews daily patient load to identify any patients on case management or in need of

Health Risk Screening.

Closes visit to ensure patient received referral along with referral instructions.

Monitors extended benefits as directed by RN or RN Supervisor.

Handles patient follow up such as Rx refills and patient questions with Primary Care Physicians.

Completes office procedures in alignment with skill sets.

Notifies RN or RN Supervisor about patient problems or complaints.

Enter referrals and appointments into IDX.

Assists in gathering information for referrals and care plans.

Limited work experience.

Works on simple tasks using established procedures.

Depends on others for guidance.

Work is typically reviewed by others.


Qualifications:


Requirements:


HS Diploma/GED

1+ year experience in a medical office

1+ year experience in an office setting where you are using the phone and computer as the primary instruments to perform the role

2+ years experience MS Office (Word, Excel and Outlook)

Must have reliable transportation

Assets:


Medical terminology/coding experience

Bilingual - English/Spanish
