AMC has a 96.5% quality rating (which exceeds state and national averages) and is a regional Level I Trauma Center that receives patients from 67 other hospitals.


The Social Worker plans, coordinates, and implements social services being intrinsic to the process of Case Management, which seeks to service efficiently its customers by providing optimum care in the most effective manner. Some of the responsibilities include:


1. Assesses psycho-social needs of patients, taking into consideration physical, emotional, social, financial, and environmental factors

2. Completes specific plan of intervention by next working day. All plans are developed in consultation with patient, family, case management team, physician, and other members of the health care team

3. Assures that significant emotional needs are addressed by the health care team or other mental health professional.

4. Makes referrals to appropriate sources for ongoing counseling or mental health treatment

5. Implements plans in a manner that will result in a timely discharge from the hospital

6. Works cooperatively with community agencies, assuring that information needed for continuity of care is provided in accordance with hospital policy on confidentiality

7. Completes social histories on selected cases, as needed

8. Provides short-term counseling to selected patients and families to help them deal with major lifestyle changes or life threatening illnesses

9. Documents initial assessment/ plan by next working day after receiving referral. Documentation clearly reflects the initial assessment and the plan for intervention

10. Documents all significant contacts in a clear, concise, timely manner


Qualifications


Position requires Bachelor's degree in Social Work and two years related experience in a hospital or medical setting. Licensure and MSW degree preferred.


The successful candidate will have:


1. Knowledge of principles and practice of social work.

2. Knowledge of federal and state laws or regulations pertaining to social services and public assistance programs.

3. Knowledge of group and family dynamics, community systems, welfare, and child development.

4. Knowledge of and adherence to National Association of Social Worker's Code of Ethics.

5. Understanding of psychosocial aspects of illness and disability.

6. Understanding of medical terminology and processes.

7. Ability to intervene in problematic relationships to provide coordination with medical team.

8. Ability to maintain effective working relationships with fellow employees, patients, families, and physicians
