Job Description:
Referral Coordinator for the In-patient and Out-patient side of Utilization Management-Medicare-Medical knowledge a must.
Responsible for coordinating and implementing in-patient utilization, plans in collaboration with Case Managers/Social Workers on an inpatient basis.
Prepare documents/chart copies/Personal Healthcare Records to accompany patients during stay and at discharge. Arrange any medical schedules for patients during inpatient time. Issue important messages from Medicare to appropriate patients during in-patient stay and prior to discharge according to dept.
Requirements:
Medi-Cal/Medicare. Assist with daily work flow tasks within the department. Documented activities. Communicate with Case Managers, Physicians, Nurses and ancillary staff on all issues. Collaborate with outside agencies to ensure seamless transitions of patients.
